All four drugs are TNF blockers which block the action of a substance in your body called TNF-alpha. TNF-alpha (tumor necrosis factor alpha) is made by your body’s immune system. What are the harmful side effects to be cautious of?

1. INFECTIONS TNF blockers decrease the body’s ability to fight infection, thus increasing the chances that a patient will develop a serious and potentially deadly infection. Some patients have died when the bacteria that cause infections have spread throughout their body (sepsis). Serious infections include aspergillosis, candidiasis and tuberculosis. On September 4, 2008, the FDA has received reports of patients developing pulmonary and disseminated histoplasmosis, coccidioidomycosis, blastomycosis and other opportunistic infections while taking TNF blockers. Signs and Symptoms of possible systemic fungal infection:

• fever
• malaise
• weight loss
• sweats
• cough
• dyspnea
• pulmonary infiltrates
• serious systemic illness with or without concomitant shock

Signs of Tuberculosis include:

• chest pain
• difficulty breathing
• cough
• sore throat
• flu-like symptoms
• weight loss
• loss of appetite
• night sweats

(Before administering TNF blocker, physicians should perform a skin test on a patient to ensure they do not have an inactive TB infection) Patients with the following risk factors are more likely to suffer a serious infection:

MALIGNANCIES There have been cases of certain kinds of cancer in patients taking TNF blockers with rheumatoid arthritis, especially more serious rheumatoid arthritis, may have a higher chance for getting a kind of cancer called lymphoma. Some patients on TNF blockers have developed types of cancer called non-melanoma skin cancer (basal call cancer and squamous call cancer) which are generally not life threatening if treated. What does the FDA say?

The Food and Drug Administration ordered stronger warnings September 4, 2008 on the four medications (Humira, Enbrel, Cimzie, Remecade) widely used to treat rheumatoid arthritis and other serious illnesses, saying they can raise the risk of possibly fatal fungal infections.
